Survodutide Dosage Protocol

BI 456906 · investigational GLP-1/glucagon dual agonist

The published phase 2 schedule starts at 0.3 mg once weekly for 2 weeks, then rises in two-week steps to 6 mg from week 23. With 1 mL of bacteriostatic water, a 10 mg vial is 10 mg/mL, so the 0.3 mg start amount is 3 units on a U-100 syringe. The protocol used a slower escalation to help manage gastrointestinal effects.

Once weekly · same day each week · subcutaneous injection

This is the published 6 mg phase 2 escalation: each step was given once weekly for 2 weeks.

The trial protocol allowed dose adjustment for adverse effects; do not treat this research schedule as individual medical advice.

02 · HOW TO MIX

Mixing a 10 mg vial with 1 mL of BAC water

Using a 1 mL (100-unit) insulin syringe, one full draw of bacteriostatic water gives 10 mg/mL.

  1. Bacteriostatic water running slowly down the inside wall of a peptide vial1

    Add the water slowly

Wipe both rubber stoppers with an alcohol swab and let them dry. Draw 1 mL of bacteriostatic water and let it run slowly down the inside of the vial wall — not straight onto the powder.

  2. Hand gently swirling an upright vial in a small circle2

    Swirl gently

Swirl or roll the vial gently until the powder has dissolved. Give it a few minutes to settle if needed.

  3. A vial being shaken with a red cross through it3

    Don't shake it

Shaking can damage the peptide and cause foaming. Gentle swirling only.

  4. Close-up of a vial with small undissolved particles and a caution symbol4

    Check it's fully clear

Only use it once it is fully clear with no particles. Label the vial with the date and 10 mg/mL, then store it in the fridge at 2–8 °C.

How it works

Survodutide, also called BI 456906, is an investigational dual agonist of the glucagon receptor and GLP-1 receptor being developed by Boehringer Ingelheim for obesity and metabolic liver disease.

It activates two receptors involved in metabolic regulation. The developer describes GLP-1 activity as affecting appetite and fullness, while glucagon-receptor activity is thought to affect liver fat and energy metabolism; these proposed clinical effects remain under investigation.

Illustrated mechanism explainer

What the research shows

  • In the obesity phase 2 study, 386 participants received treatment for 46 weeks, including 20 weeks of escalation and 26 weeks of maintenance. Planned-treatment mean body-weight change at week 46 was −14.9% with 4.8 mg versus −2.8% with placebo.
  • That phase 2 obesity study tested 0.6, 2.4, 3.6 and 4.8 mg once weekly; adverse events occurred in 91% of survodutide recipients and gastrointestinal adverse events in 75%.
  • In the 48-week phase 2 MASH study of 293 participants, MASH improvement without worsening of fibrosis occurred in 47% at 2.4 mg, 62% at 4.8 mg and 43% at 6 mg, versus 14% with placebo.
  • In a 76-week phase 3 obesity study, mean body-weight change was −12.2% with 3.6 mg and −13.0% with 6 mg, compared with −5.4% with placebo. Survodutide remains investigational and has not been approved for use.
  • The TGA says the ARTG is the database of therapeutic goods approved for supply in Australia; no Survodutide or BI 456906 entry was located in the ARTG search checked for this guide.

Reported side effects

  • Gastrointestinal effects were the main reported adverse events. In phase 2 obesity, 232 of 309 survodutide recipients (75%) had gastrointestinal adverse events, compared with 32 of 77 placebo recipients (42%).
  • In the phase 2 MASH study, nausea occurred in 66% with survodutide versus 23% with placebo, diarrhoea in 49% versus 23%, and vomiting in 41% versus 4%.
  • The MASH study reported serious adverse events in 8% with survodutide and 7% with placebo. Human safety and long-term effects are still being evaluated.

Important notes

  • Survodutide is an investigational medicine and is not approved for use in Australia or the United States. It is not stocked by Peak Peptides Australia.
  • Prohibited in sport at all times: because it has no current governmental regulatory approval for human therapeutic use, it falls within WADA's S0 non-approved-substances class.
  • Published study material excluded people with a personal or family history of medullary thyroid carcinoma or multiple endocrine neoplasia syndrome type 2, and people with a history of pancreatitis; this is not a substitute for clinical assessment.
  • The main phase 2 risks were nausea, diarrhoea and vomiting, particularly during dose escalation. Seek urgent clinical care for severe or persistent symptoms.
  • Never use a mixed vial that is cloudy or has particles in it.

Injection basics

  • Use a new sterile 1 mL (100-unit) U-100 insulin syringe, 27–31G, every time — never reuse or share.
  • Wipe the vial stopper and the skin with an alcohol swab and let both dry first.
  • Inject under the skin (subcutaneously) of the abdomen, thigh or upper arm, and rotate sites.
  • Put used syringes straight into an approved sharps container — the NDSS lists Australian disposal options.

06 · QUICK ANSWERS

Survodutide questions

How many units is 300 mcg of Survodutide?

With the default 10 mg vial plus 1 mL (10 mg/mL), 300 mcg is 3 units, or 0.03 mL, on a U-100 syringe.

How many units is 6 mg of Survodutide?

With the default 10 mg/mL mix, 6 mg is 60 units, or 0.60 mL, on a U-100 syringe.

How much bacteriostatic water is used for a 10 mg Survodutide vial?

This reference guide uses 1 mL for a 10 mg/mL concentration. At that concentration, 1 mg is 10 units, or 0.10 mL, on a U-100 syringe.

How long does a 10 mg Survodutide vial last?

At 0.3 mg once weekly, 10 mg contains about 33.3 weekly doses. At 4.8 mg it contains about 2.1 weekly doses, and at 6 mg it contains about 1.7 weekly doses.
