Cagrilintide Dosage Protocol

Long-acting amylin analogue · CagriSema component

The published 4.5 mg target schedule used 0.6 mg once weekly for weeks 1–2, then 1.2 mg for weeks 3–4 and 2.4 mg for weeks 5–6 before 4.5 mg weekly. With 1 mL in Peak's 5 mg vial, the concentration is 5 mg/mL, so the 0.6 mg starting amount is 12 U-100 units (0.12 mL).

The published 4.5 mg target schedule changed amount every 2 weeks through the first 6 weeks, then continued at 4.5 mg once weekly.

The study permitted a one-week postponement of an increase for intolerable gastrointestinal symptoms; 4.5 mg was the highest amount studied in this chart source.

These are milligram amounts from a supervised research schedule; the 5 mg vial determines only the concentration and volume arithmetic.

This chart reproduces an investigational clinical-trial escalation; cagrilintide is not an approved medicine. Education only, not medical advice. Dose amounts adapted from Supplementary appendix: cagrilintide phase 2 escalation regimen (Lau et al., Lancet 2021); units are recalculated for the vial and mix selected above.

02 · HOW TO MIX

Mixing a 5 mg vial with 1 mL of BAC water

Using a 1 mL (100-unit) insulin syringe, one full draw of bacteriostatic water gives 5 mg/mL.

  1. Bacteriostatic water running slowly down the inside wall of a peptide vial1

    Add the water slowly

Wipe both rubber stoppers with an alcohol swab and let them dry. Draw 1 mL of bacteriostatic water and let it run slowly down the inside of the vial wall — not straight onto the powder.

  2. Hand gently swirling an upright vial in a small circle2

    Swirl gently

Swirl or roll the vial gently until the powder has dissolved. Give it a few minutes to settle if needed.

  3. A vial being shaken with a red cross through it3

    Don't shake it

Shaking can damage the peptide and cause foaming. Gentle swirling only.

  4. Close-up of a vial with small undissolved particles and a caution symbol4

    Check it's fully clear

Only use it once it is fully clear with no particles. Label the vial with the date and 5 mg/mL, then store it in the fridge at 2–8 °C.

How it works

Cagrilintide is a long-acting analogue of amylin, a pancreatic hormone that contributes to meal-size and satiety signalling.

Amylin signalling is thought to reduce food intake, slow gastric emptying and suppress the rise in glucagon after meals. Cagrilintide has been designed for longer exposure than native amylin, which supported once-weekly research schedules.

Illustrated mechanism explainer

What the research shows

  • The primary phase 2 trial randomly assigned 706 adults with overweight or obesity without diabetes to cagrilintide dose groups, liraglutide or placebo. Treatment lasted 26 weeks, including up to 6 weeks of escalation, followed by 6 weeks without treatment.
  • In that trial, mean body-weight change at week 26 ranged from −6.0% at 0.3 mg to −10.8% at 4.5 mg, compared with −3.0% with placebo. These are group results in the study population, not an individual prediction.
  • The 4.5 mg group used 0.6 mg in weeks 1–2, 1.2 mg in weeks 3–4, 2.4 mg in weeks 5–6 and 4.5 mg from week 7 in the published supplementary escalation table.
  • Cagrilintide is investigational rather than a TGA-registered medicine in Australia. The FDA states that cagrilintide is not a component of an FDA-approved drug and has not been found safe and effective for any condition.
  • CagriSema is a combination of cagrilintide and semaglutide; results for the combination do not establish effects or dosing for cagrilintide alone.

Reported side effects

  • In the phase 2 trial, gastrointestinal adverse events occurred in 41%–63% across cagrilintide groups versus 32% with placebo. Nausea occurred in 20%–47% versus 18% with placebo.
  • The most frequent events in the trial were nausea, constipation and diarrhoea, together with administration-site reactions.
  • Across all treatment groups in the trial, 73 participants (10%) permanently discontinued treatment; 30 participants (4%) did so because of adverse events.
  • There is no approved product label establishing cagrilintide's full safety profile, contraindications or long-term risks.

Important notes

  • Cagrilintide is not a TGA-registered medicine in Australia. A research vial is not interchangeable with the pen formulation and monitoring used in the published study.
  • The 2026 WADA Prohibited List places pharmacological substances with no current approval by any governmental regulatory health authority for human therapeutic use in S0, prohibited at all times. This applies to investigational cagrilintide.
  • Gastrointestinal symptoms were common in the published study. Do not transfer doses, safety claims or combination results from semaglutide, tirzepatide or CagriSema to cagrilintide alone.
  • The FDA warns that unapproved products have not undergone FDA review for safety, effectiveness or quality; a supplier's nominal vial strength does not verify identity, sterility or concentration.
  • Never use a mixed vial that is cloudy or has particles in it.

Injection basics

  • Use a new sterile 1 mL (100-unit) U-100 insulin syringe, 27–31G, every time — never reuse or share.
  • Wipe the vial stopper and the skin with an alcohol swab and let both dry first.
  • Inject under the skin (subcutaneously) of the abdomen, thigh or upper arm, and rotate sites.
  • Put used syringes straight into an approved sharps container — the NDSS lists Australian disposal options.

06 · QUICK ANSWERS

Cagrilintide questions

How many units is the 0.6 mg Cagrilintide starting amount?

With the default 1 mL mix in a 5 mg vial, the concentration is 5 mg/mL. A 0.6 mg amount is 0.12 mL, which is 12 units on a U-100 syringe.

How many units are the published weekly Cagrilintide steps?

At 5 mg/mL from a 5 mg vial plus 1 mL, 1.2 mg is 24 units (0.24 mL), 2.4 mg is 48 units (0.48 mL), and 4.5 mg is 90 units (0.90 mL) on a U-100 syringe.

How much water is used for the 5 mg Cagrilintide calculation?

The default calculator mix is 1 mL in the 5 mg vial, giving 5 mg/mL or 50 mcg per U-100 unit. The opened Peak page did not state a product-specific diluent or final volume, so this is concentration arithmetic based on the opened 5 mg reference page, not a supplier instruction.

How long does a 5 mg Cagrilintide vial last?

By mass arithmetic, a 5 mg vial contains eight complete 0.6 mg weekly amounts, four complete 1.2 mg amounts, two complete 2.4 mg amounts, or one complete 4.5 mg amount with 0.5 mg left. The displayed 26-week schedule totals 98.4 mg, so it requires 20 nominal 5 mg vials after rounding up from 19.68.
