Gonadorelin Dosage Protocol

GnRH · gonadotropin-releasing hormone decapeptide

The reference protocol lists 50 mcg per injection 2–3 times weekly for weeks 1–2, 100 mcg for weeks 3–4, then 100–150 mcg for weeks 5–8. With 2 mL in a 2 mg vial, the concentration is 1 mg/mL, so the 50 mcg starting amount is 5 units (0.05 mL) on a U-100 syringe.

The attributed protocol uses 2–3 injections per week on non-consecutive days.

Its stated standard course is 4–8 weeks; the 100–150 mcg range is used only in weeks 5–8.

Clinical pulsatile GnRH treatment uses specialist-supervised pumps and is not this intermittent chart.

02 · HOW TO MIX

Mixing a 2 mg vial with 2 mL of BAC water

Using a 1 mL (100-unit) insulin syringe, 2 full draws of bacteriostatic water gives 1 mg/mL.

  1. Bacteriostatic water running slowly down the inside wall of a peptide vial1

    Add the water slowly

Wipe both rubber stoppers with an alcohol swab and let them dry. Draw 2 mL of bacteriostatic water and let it run slowly down the inside of the vial wall — not straight onto the powder.

  2. Hand gently swirling an upright vial in a small circle2

    Swirl gently

Swirl or roll the vial gently until the powder has dissolved. Give it a few minutes to settle if needed.

  3. A vial being shaken with a red cross through it3

    Don't shake it

Shaking can damage the peptide and cause foaming. Gentle swirling only.

  4. Close-up of a vial with small undissolved particles and a caution symbol4

    Check it's fully clear

Only use it once it is fully clear with no particles. Label the vial with the date and 1 mg/mL, then store it in the fridge at 2–8 °C.

How it works

Gonadorelin is a synthetic form of gonadotropin-releasing hormone, a decapeptide signal from the hypothalamus. It acts at GnRH receptors on the anterior pituitary and can trigger release of luteinising hormone and follicle-stimulating hormone.

The timing matters. Pulsatile GnRH signalling stimulates LH and FSH release, whereas continuous GnRH agonist stimulation can desensitise pituitary signalling and suppress gonadotropin secretion.

Illustrated mechanism explainer

What the research shows

  • FDA’s orphan-drug database records historical US marketing approval of gonadorelin acetate (Lutrepulse) on 10 October 1989 for induction of ovulation in women with hypothalamic amenorrhoea caused by deficient or absent endogenous GnRH secretion. That record does not validate a 2 mg research vial or this intermittent chart.
  • In a multicentre report of 109 women with hypothalamic amenorrhoea receiving intravenous pulsatile gonadorelin, ovulation occurred in 91% of those with primary and 96% with secondary hypothalamic amenorrhoea. This is specialised intravenous pulsatile treatment, not the subcutaneous reference-site schedule.
  • A non-randomised study in 28 men with congenital hypogonadotropic hypogonadism compared a subcutaneous pulsatile gonadorelin pump in 10 men with gonadotropin therapy in 18. Spermatogenesis occurred in 90% and 83.3%, respectively, over up to 24 months; the pump began at 10 mcg every 90 minutes.
  • The TGA says the ARTG is the public database for therapeutic goods that can be legally supplied in Australia. No ARTG number or Australian prescription product for this 2 mg research vial was supplied or verified, so it must not be represented as a TGA-approved medicine.

Reported side effects

  • In the 28-person pump study, skin red induration was reported in 9 of 10 pump participants and a micro-abscess occurred in one; these were pump-needle-site effects in a very different regimen from this chart.
  • In the 109-woman intravenous pulsatile-treatment report, line-placement complications ranged from 0% to 11% across centres, with a 7% mean; multiple pregnancy occurred in 12% and one ovarian hyperstimulation case was reported in a participant also receiving clomiphene citrate.
  • A published paediatric case report describes life-threatening anaphylaxis within 3 minutes of intravenous gonadorelin acetate. It is rare, but hypersensitivity can be serious.

Important notes

  • Gonadorelin has hormonal effects on LH and FSH. Fertility, testosterone and other endocrine concerns need clinician assessment rather than self-treatment.
  • FDA’s orphan-drug database records a historical US approval for Lutrepulse in a specific ovulation indication; that does not establish TGA approval or quality for a 2 mg research vial in Australia.
  • Prohibited in sport at all times: the 2026 WADA Prohibited List names gonadotrophin-releasing hormone, including gonadorelin, among testosterone-stimulating peptides in S2.2.1.
  • Continuous GnRH agonist exposure can desensitise pituitary signalling; do not substitute this intermittent reference chart for a prescribed pulsatile treatment plan.
  • Never use a mixed vial that is cloudy or has particles in it.

Injection basics

  • Use a new sterile 1 mL (100-unit) U-100 insulin syringe, 27–31G, every time — never reuse or share.
  • Wipe the vial stopper and the skin with an alcohol swab and let both dry first.
  • Inject under the skin (subcutaneously) of the abdomen, thigh or upper arm, and rotate sites.
  • Put used syringes straight into an approved sharps container — the NDSS lists Australian disposal options.

06 · QUICK ANSWERS

Gonadorelin questions

How many units is 50 mcg of gonadorelin from a 2 mg vial?

With the default 2 mL mix, a 2 mg vial is 1 mg/mL. A 50 mcg amount is 5 units, which is 0.05 mL, on a U-100 syringe.

How many units are 100 mcg and 150 mcg of gonadorelin?

With the default 2 mg plus 2 mL mix, 100 mcg is 10 units (0.10 mL) and 150 mcg is 15 units (0.15 mL) on a U-100 syringe.

How much BAC water is used for a 2 mg gonadorelin vial?

The attributed 2 mg reference protocol uses 2 mL of bacteriostatic water. This makes 1 mg/mL, so each U-100 unit contains 10 mcg.

How long does a 2 mg gonadorelin vial last?

At 100 mcg per injection, a 2 mg vial contains 20 injections and lasts about 6.7 weeks at 3 injections weekly. The stepped schedule and the 2–3 weekly range change the actual supply needed.
