FOXO4-DRI Dosage Protocol

D-retro-inverso senolytic peptide · FOXO4–p53 interaction blocker

The attributed educational schedule uses 250 mcg once daily in weeks 1–4, 375 mcg in weeks 5–8, then 500 mcg in weeks 9–16. With 3 mL of bacteriostatic water, a 10 mg vial is 3.33 mg/mL, so the 250 mcg start amount is 7.5 units (0.075 mL) on a U-100 syringe.

02 · HOW TO MIX

Mixing a 10 mg vial with 3 mL of BAC water

Using a 1 mL (100-unit) insulin syringe, 3 full draws of bacteriostatic water gives 3.33 mg/mL.

  1. Bacteriostatic water running slowly down the inside wall of a peptide vial1

    Add the water slowly

Wipe both rubber stoppers with an alcohol swab and let them dry. Draw 3 mL of bacteriostatic water and let it run slowly down the inside of the vial wall — not straight onto the powder.

  2. Hand gently swirling an upright vial in a small circle2

    Swirl gently

Swirl or roll the vial gently until the powder has dissolved. Give it a few minutes to settle if needed.

  3. A vial being shaken with a red cross through it3

    Don't shake it

Shaking can damage the peptide and cause foaming. Gentle swirling only.

  4. Close-up of a vial with small undissolved particles and a caution symbol4

    Check it's fully clear

Only use it once it is fully clear with no particles. Label the vial with the date and 3.33 mg/mL, then store it in the fridge at 2–8 °C.

How it works

FOXO4-DRI is a synthetic D-retro-inverso peptide designed to disrupt binding between FOXO4 and p53. In cellular experiments, this released active p53 from the nucleus and activated caspase-dependent apoptosis in senescent cells.

The original research and later keloid work tested this idea in cells, organ cultures and mice. These preclinical findings do not show that selectively clearing senescent cells is effective or safe in people.

Illustrated mechanism explainer

What the research shows

  • In the original 2017 mouse chemotoxicity model, FOXO4-DRI was given intravenously at 5 mg/kg on days 1, 3 and 5 after doxorubicin; the findings were in mice, not people.
  • A 2020 study in naturally aged 20–24-month-old male mice used 5 mg/kg intraperitoneally every other day for 3 administrations. Serum testosterone was significantly higher 30 days later; the reported group size was 6.
  • A 2024 aged-mouse paper reported increased sperm quality and improved spermatogenesis after FOXO4-DRI treatment, while a 2025 paper reported apoptosis in keloid fibroblast and organ-culture models. Neither is human dosing or efficacy evidence.
  • The ClinicalTrials.gov FOXO4-DRI search opened for this guide returned no studies. The 250–500 mcg chart amounts therefore do not come from a human clinical dose-escalation.
  • An ARTG search for FOXO4-DRI returned no matching entry, and the DailyMed SPL API returned zero labels. FOXO4-DRI should not be presented as a TGA- or FDA-approved medicine.

Reported side effects

  • The opened primary sources provide no human adverse-event rates, so human safety, interactions and contraindications are unknown.
  • The cited experiments are in cells, organ cultures and mice; their results cannot establish the tolerability of 250–500 mcg given subcutaneously in people.
  • FOXO4-DRI is intended to trigger apoptosis in experimental senescent cells, but possible off-target effects in humans have not been characterised.

Important notes

  • FOXO4-DRI is not an approved medicine in Australia: the ARTG search opened for this guide returned no matching record.
  • Competitive athletes should treat it as prohibited at all times: WADA S0 prohibits pharmacological substances with no current governmental approval for human therapeutic use.
  • There is no clinically established human dose, efficacy or safety profile; do not use preclinical senolytic findings as evidence of treatment benefit.
  • The reference chart is not comparable with the 5 mg/kg intravenous or intraperitoneal animal regimens in the published studies.
  • Never use a mixed vial that is cloudy or has particles in it.

Injection basics

  • Use a new sterile 1 mL (100-unit) U-100 insulin syringe, 27–31G, every time — never reuse or share.
  • Wipe the vial stopper and the skin with an alcohol swab and let both dry first.
  • Inject under the skin (subcutaneously) of the abdomen, thigh or upper arm, and rotate sites.
  • Put used syringes straight into an approved sharps container — the NDSS lists Australian disposal options.

06 · QUICK ANSWERS

FOXO4-DRI questions

How many units is 250 mcg of FOXO4-DRI?

With 3 mL in a 10 mg vial (3.33 mg/mL), 250 mcg is 7.5 units, or 0.075 mL, on a U-100 syringe.

How much bacteriostatic water is used for a 10 mg FOXO4-DRI vial?

The attributed 10 mg reference protocol uses 3 mL, making 3.33 mg/mL. At that concentration, 375 mcg is 11.25 units (0.1125 mL) and 500 mcg is 15 units (0.15 mL) on a U-100 syringe.

How long does a 10 mg FOXO4-DRI vial last?

Ignoring preparation losses, 10 mg provides 40 once-daily 250 mcg amounts, 26.67 once-daily 375 mcg amounts, or 20 once-daily 500 mcg amounts.
