Thymosin Alpha-1 Dosage Protocol

Thymalfasin · 28-amino-acid immunomodulatory peptide

The reference protocol uses 300 mcg once daily in week 1, then 500 mcg once daily from week 2. With 3 mL of bacteriostatic water, a 10 mg vial is 3.33 mg/mL, so the 300 mcg starting amount is 9 units on a U-100 syringe. The 500 mcg amount is 15 units.

02 · HOW TO MIX

Mixing a 10 mg vial with 3 mL of BAC water

Using a 1 mL (100-unit) insulin syringe, 3 full draws of bacteriostatic water gives 3.33 mg/mL.

  1. Bacteriostatic water running slowly down the inside wall of a peptide vial1

    Add the water slowly

Wipe both rubber stoppers with an alcohol swab and let them dry. Draw 3 mL of bacteriostatic water and let it run slowly down the inside of the vial wall — not straight onto the powder.

  2. Hand gently swirling an upright vial in a small circle2

    Swirl gently

Swirl or roll the vial gently until the powder has dissolved. Give it a few minutes to settle if needed.

  3. A vial being shaken with a red cross through it3

    Don't shake it

Shaking can damage the peptide and cause foaming. Gentle swirling only.

  4. Close-up of a vial with small undissolved particles and a caution symbol4

    Check it's fully clear

Only use it once it is fully clear with no particles. Label the vial with the date and 3.33 mg/mL, then store it in the fridge at 2–8 °C.

How it works

Thymosin Alpha-1, also called thymalfasin, is a synthetic 28-amino-acid peptide. It is studied as an immunomodulator rather than as an antimicrobial drug.

FDA briefing material describes immune effects mediated in part through Toll-like receptor 9 on dendritic and lymphoid progenitor cells. Animal research has reported changes in immune responses, but those findings do not establish a benefit from this research vial.

Illustrated mechanism explainer

What the research shows

  • A review of four chronic hepatitis B clinical trials covering 195 people reported HBV DNA clearance of 40.6% after 6 months and 25.6% after 12 months in one randomised trial, versus 9.4% in untreated controls; the review says results across hepatitis studies were mixed.
  • In the 2025 TESTS sepsis trial, 1,106 adults were enrolled and 1,089 were analysed. Twenty-eight-day mortality was 23.4% with thymosin alpha-1 and 24.1% with placebo (hazard ratio 0.97, 95% CI 0.76–1.24).
  • In TESTS, the treatment was a subcutaneous injection every 12 hours for up to 7 days; no measured safety outcome differed statistically significantly between groups.
  • FDA states it has approved no drug products containing thymosin alpha-1. The TGA ARTG is Australia’s public database for goods approved for supply; this Peak item is sold as a research sample, not as an ARTG-listed medicine.

Reported side effects

  • FDA’s review says the most commonly reported reactions in clinical studies were local irritation, redness and injection-site discomfort.
  • FDA notes reported alanine aminotransferase flares in chronic hepatitis B studies and thyroid-stimulating-hormone abnormalities in chronic hepatitis C studies.
  • FDA identifies potential immunogenicity concerns for injectable preparations because of aggregation and peptide-related impurities; these risks may differ by formulation and quality controls.
  • In the 1,089-person TESTS analysis, no safety outcome differed statistically significantly between the thymosin alpha-1 and placebo groups, but that finding applies to the studied medicine and setting.

Important notes

  • This 10 mg Peak product is a research sample, not a TGA-approved Australian medicine; US FDA has approved no drug products containing thymosin alpha-1.
  • The 2026 WADA Prohibited List names Thymosin-beta4 and TB-500, not Thymosin Alpha-1 or thymalfasin. Athletes should check the current list and their sport’s rules before using any peptide.
  • Clinical findings from finished thymalfasin medicines do not establish the identity, quality, dose or effects of an online research vial.
  • Never use a mixed vial that is cloudy or has particles in it.

Injection basics

  • Use a new sterile 1 mL (100-unit) U-100 insulin syringe, 27–31G, every time — never reuse or share.
  • Wipe the vial stopper and the skin with an alcohol swab and let both dry first.
  • Inject under the skin (subcutaneously) of the abdomen, thigh or upper arm, and rotate sites.
  • Put used syringes straight into an approved sharps container — the NDSS lists Australian disposal options.

06 · QUICK ANSWERS

Thymosin Alpha-1 questions

How many units is 300 mcg of Thymosin Alpha-1?

With 3 mL in a 10 mg vial, the concentration is 3.33 mg/mL. A 300 mcg amount is 9 units, or 0.09 mL, on a U-100 syringe.

How many units is 500 mcg of Thymosin Alpha-1?

With 3 mL in a 10 mg vial, 500 mcg is 15 units, or 0.15 mL, on a U-100 syringe.

How much BAC water is used for a 10 mg Thymosin Alpha-1 vial?

The default mix is 3 mL of BAC water in a 10 mg vial. That calculation gives 3.33 mg/mL, where 30 units on a U-100 syringe equals 1 mg.

How long does a 10 mg Thymosin Alpha-1 vial last?

At 300 mcg once daily, 10 mg contains about 33.3 daily amounts. At 500 mcg once daily, it contains 20 daily amounts.
