How it works

MK-677, also called ibutamoren mesylate, is an orally active, non-peptide growth hormone secretagogue. It was developed to have oral exposure suitable for once-daily administration.

It activates the growth hormone secretagogue receptor, the receptor for ghrelin. In human research, it enhanced pulsatile growth-hormone release and raised IGF-1; these effects do not establish a therapeutic benefit for unapproved use.

What the research shows

In a two-year randomised crossover study, 65 healthy adults aged 60–81 received 25 mg oral MK-677 or placebo once daily; the main comparison was after 12 months.
At 12 months, fat-free mass changed by 1.1 kg with MK-677 versus −0.5 kg with placebo, while abdominal visceral fat and total fat mass did not differ significantly.
In that study, fasting blood glucose increased by an average 0.3 mmol/L (5 mg/dL) with MK-677 and insulin sensitivity declined.
A separate study of 123 older adults recovering from hip fracture used 25 mg/day for up to 24 weeks; it ended early after a congestive-heart-failure safety signal in a limited number of participants.
The TGA states that no product containing ibutamoren has been approved for supply in Australia.

Reported side effects

In the healthy-older-adult study, increased appetite was reported by 29 of 43 people given MK-677 (67%) versus 8 of 22 given placebo (36%).
Mild transient lower-leg oedema was reported by 19 of 43 people given MK-677 (44%) versus 6 of 22 given placebo (27%).
Transient muscle pain was reported by 14 of 43 people given MK-677 (33%) versus 2 of 22 given placebo (9%).
The hip-fracture study was terminated early because of a congestive-heart-failure safety signal, so this risk needs particular clinical attention in vulnerable people.

Important notes

MK-677 is not listed on the ARTG, and the TGA says no product containing ibutamoren has been approved for supply in Australia.
Prohibited in sport: WADA lists ibutamoren (MK-677) as a growth hormone secretagogue under S2.2.4, prohibited at all times.
The TGA tested one unapproved MK-677 capsule product labelled as 15 mg ibutamoren and found undeclared metandienone, an anabolic steroid.
Human research found increased fasting glucose and reduced insulin sensitivity; a 25 mg/day hip-fracture study also stopped early after a congestive-heart-failure safety signal.
