Tirzepatide Reconstitution and Dosing Guide

An evidence-aligned Tirzepatide guide explaining the four-week initiation phase, maintenance-dose selection, and why dose increases are not automatic.

Historical research article — verify before use: Some older examples, including amounts, routes, diluents and storage windows, have not been individually rechecked against current primary sources and may not apply to any human or supplier vial. Always check the exact product's formulation, primary sources and current licensed instructions before interpreting preparation or dosing information.

What is Tirzepatide?

Tirzepatide is a dual GIP/GLP-1 receptor agonist peptide that has shown significant effects on glucose metabolism and body composition in research settings. It activates both the glucose-dependent insulinotropic polypeptide (GIP) receptor and the glucagon-like peptide-1 (GLP-1) receptor, producing effects on appetite regulation, insulin sensitivity, and metabolic rate in test subjects. It is available as a lyophilized research peptide in 5mg, 10mg, 15mg, and 30mg vials. For research purposes only.

How to Reconstitute Tirzepatide

For a 10mg vial: Add 2mL bacteriostatic water = 5mg/mL (50 mcg per unit on U-100 syringe) • 2.5mg dose = 50 units (0.5mL) • 5mg dose = 100 units (1mL) — requires full 1mL syringe For a 30mg vial: Add 2mL bacteriostatic water = 15mg/mL (150 mcg per unit) • 2.5mg dose = 16.7 units (round to 17 units) • 5mg dose = 33 units • 7.5mg dose = 50 units • 10mg dose = 67 units • 12.5mg dose = 83 units • 15mg dose = 100 units The 30mg vial with 2mL is the most cost-effective option for research protocols, as it provides multiple weeks of doses from a single reconstitution.

Standard Titration Protocol

Current official Tirzepatide prescribing information uses 2.5 mg once weekly for the first 4 weeks, followed by 5 mg once weekly. The 2.5 mg step is an initiation dose rather than a maintenance dose. After the 5 mg step, any increase is by 2.5 mg only after at least 4 weeks at the current dose. Dose increases are not automatic: the selected maintenance dose should reflect response and tolerability in the patient under clinical care. Labelled maintenance options for weight management are 5 mg, 10 mg, or 15 mg once weekly, with 15 mg as the maximum. A schedule that changes from 1 mg to 1.5 mg, 2 mg, and 2.5 mg in consecutive weeks is not the labelled Tirzepatide titration schedule. Source: https://pi.lilly.com/us/zepbound-uspi.pdf

Injection Protocol

Route: Subcutaneous injection Frequency: Once weekly, on the same day each week Timing: Any time of day, with or without food Injection sites: Abdomen, thigh, or upper arm — rotate weekly Needle: U-100 insulin syringe, 29-31 gauge Important: If a dose is missed, administer it as soon as possible within 4 days of the missed day. If more than 4 days have passed, skip the missed dose and resume on the next scheduled day. Do not double-dose.

Storage Instructions

For lyophilised and reconstituted tirzepatide research material, use the exact supplier label, formulation documentation and lot-specific COA for temperature, light protection, expiry and discard timing. Do not assume a generic research vial shares the storage profile of a commercial prefilled medicine, and do not treat bacteriostatic water as proof of a universal 28-day shelf life.
